Limp wrist does not explain how a slide passes over a properly seated loaded mag, goes fully into battery, without picking up a round in its travel and pushing it into the chamber. (loaded mag = mag with any live rounds in it)

Nor does lack of oil.
Actually yes it does. The gun generates enough recoil to extract and eject the empty but not enough to allow full recoil and the slide to run rearward enough to pick up a fresh round. The gun can eject and slide not quite go back far enough to pick up a greasy round
