Does anybody have a copy of the "fill-in" pdf form that actually works?

For refernce, all of my computers and 4 operating systems allow me to fill it in, but I can't print it without jumping through some hoops.

I'd like to hear from anyone who has actually used it, what operating system (and version), and acrobat reader version you have installed.

Windows XP Home Edition on a Dell with Acrobat version 5 and HP 960C printer. Works great. I have an older computer that I use occassionally and when it had Acrobat version 4 that form didn't print worth a darn but with version 5 does great.

Hmmmm...I see a trend. All winblows users with non-postscript and, presumably, non-networked printers....

W2k Pro, SP2, Acrobat 5 and the built-in print-to-fax. No problem with my JetDirect printer either.

If you get info for the PDF file, it will say that the PDF (the language for Acrobat files) version is 1.4, and in parentheses "Acrobat 5.x." This means that Acrobat 5.0 or greater should be used to display *and* print this file. The file *may* print fine in Acrobat 4.0 in some environments. This may seem like nitpicking, but this information comes from a reliable source (hint: I work for Adobe, and my wife is on the Acrobat development team).
