I just got issued a new jacket and want to get my name embroidered on the jacket, I am thinking about getting a piece of cloth with my name sewn on that, much like the military uses. Any cheap places to get that done?

Or there's always Army/Navy Surplus stores. Just ask for a name tape, and they should be able to hook you up.

What they said. We have ordered them from Galls in the past. If possible, try to find one that's fade-resistant...mine (black uniforms) go gray a lot faster than the uniform itself.

Velcro it if you can. It can be replaced if it fades faster than the jacket. I did mine that way and when I left the job, I sold the jacket [minus the nametag] to another employee with no hassles.

I'd say get the name strip and get it stitched on. That way if/when it fades you can replace it (stitching is easy to remove) but some schmuck who decides he wants to steal your jacket won't be able to just rip your name off. A friend left his jacket in the office and it grew legs. His name plate that was on it was sent back to him via interoffice mail.
