Did you just print the blank for and fill in by hand? I have tried the fill and print form and it wont accept half of the info. My member number comes out like a monetary value, my member expiration date is not accepted, it would only accept the first 5 digits of my social security number and a host of other problems, so I figured the online forms were not ready yet.
I did the fill in form and just left blank the portions not working right. Filled them in with the ink pen. ;f
