I have been an EMT for over 10 years, Fire, 911 ALS Ambulance and now LE. I have responded and treated multiple emergency responders, about a dozen on duty cops and COs. Motorcycle accidents, patrol car accidents, stabbings, assaults, etc. When the EMT and Medic need to know your information, they need to know it right now. I have waited for the watch commander or dispatcher to call me with the officers information, it takes too much time… critical time. I have always carried some sort of medical information sheet on me. This is what I carry now in the trauma plate pocket of my vest. It’s not as small as a wallet card but it’s easy to find, point too, read, and copy. I fill it out and make a copy for my file so I have extras when I forget and wash it with my vest carrier. I made this sheet myself, feel free to PM me and I can e-mail you the PDF file. If you find it worthy, please feel free to pass it along. Take care, David.
