    INDIVIDUAL FREEDOM MANDATE

I, __________________________, freely choose to not buy health insurance for myself or my family.

    DO NOT RESUSCITATE

If I, or any of my family members, lose consciousness, stop breathing, and require emergency care for any reason.

    DO NOT TREAT ME /CENTER]

or any member of my family members for any medical emergency if the total bill would likely exceed ___________dollars.

I am a free individual responsible for my own decisions

Do you really not want a member of your family resuscitated just because it might cost more than a certain amount of money?

    What amount would that be?
How much do you think your family members are worth?

You have missed the message. Of course, our medical system is going to resuscitate and treat whether or not one has medical insurance and that would be the desire of every caring family member. That's the point. We are all in this together when it comes to medical expenses since all of us who choose to be responsible enough to buy insurance end up paying for those who don't. That's why the individual mandate was an idea that appealed so much to conservatives until a Democratic Congress incorporated it in the Affordable Care Act and it became associated with Obama's name.
