Engraving question

    I am getting ready to complete my first form 1 for an sbr and a form for for my first aow and I have a couple of questions.

    1. What exactly do I need to have engraved on my lower, first and last name on the first line and city and state on the second line. Or do I need my full name including my middle name.

    2. I plan on sending in both my form 1 and 4 at the same time, o I still need 2 seperate finger print cards.

    3. If there a certain finger print card tht needs to be used and can ismail my form 1 and form 4 in the same envelope.

    1. If you search this forum, you can find the answer to your first question: First MI Last, and City, State.

    2. Yes.

    3a. There is usually a preprinted ATF one to use included with your Form 4 packet from the dealer. 3b. I would not recommend it.
